[SystemSafety] Failure Modes in ML

Many of the failure modes listed appear to be those of machine-learning
based software components; others are those of the processes/steps used to
develop those components and would be potential causes or mechanisms for
the former.
Is it common to have a failure modes identification combine the two
together?
