Lots of early papers cite one paper as the
source for their reliability data
J. D. Musa, "Software Reliability Data", 1979
Data and Analysis Center for Software,
Rome Air Development Center, Rome, New York.
I cannot find a copy of this report. Does anybody have
a copy they would be happy sharing?
I have checked

and obviously Google and archive.org
Some of the data is available via researchers including
it in the report they published.
There are also some tables in Musa's 1987 book
"Software reliability" (later editions have seriously watered
down the technical contents, and lack most of the tables).
